Pre-Register Visit

You don’t have to have an appointment to be seen at UFC and can just walk in. However, to shorten your waiting time you can Pre-Register your Visit Here. The time you choose is an ESTIMATED TIME. Delays may arise if UFC is unexpectedly busy.

Please bring with you the following to your visit:

Insurance Card
Photo Identification
Your Co-Pay (we accept cash, check or credit card)
Your facemask. All patients are required to wear facemasks and maintain social distancing at all times.

IMPORTANT NOTICE:

All patients with any COVID-19 symptom or who have been exposed to COVID-19 have to have a scheduled visit. Upon arrival at UFC call 865-288-7777. Do not use the UFC main entrance. We have designated separate exam rooms that are directly accessible from the parking lot. These patients must wait in their cars until one of the exam rooms is disinfected and ready. We take extreme measures to protect our patients and staff from COVID-19 and we appreciate your cooperation!
